- Mechanical failure or malfunction. This can include faulty design, testing, manufacture, warnings, instructions, quality control, and installation.
- Electrical malfunction. A helicopter’s electrical systems can malfunction for a variety of reasons, including product defects, short circuits, and corrosion.
